Leave Request Form

The FSX Fleet leave request form is used when members can not be active for a period of 2 weeks or longer. Officers may fill this form out if the period of inactivity is less then 2 weeks but are not required to. FSX Fleet members who are inactive for a period of 4 weeks that have not filled out the leave request form will be taken off the FSX Fleet Roster.

 

Special Position Request Form

The Special Position Request Form is for either FSX Fleet members or people interested in applying for the FSX Fleet that want to help out in the FSX Fleet. Officers must fill out the form correctly and will be emailed if accepted. Accepted members may receive an early stage promotion if they excel in the position they have been accepted for. Note: Not everyone is accepted. Members accepted will be emailed at the email address provided on the form.

 

Transfer Request Form

The FSX Fleet Transfer Request Form is used to request to be transferred to different squadrons, MAJCOMs, or Branches. Most members will not be transferred but if a member is accepted to be transferred they will receive an email regarding this. All transfers must be approved by the members MAJCOM commander unless the MAJCOM commander is involved in the reason for this transfer.

Training Request Forms

FTS Training Request Form

Members who have recently been assigned to a fighter squadron may request training by filling out the FTS Training Request Form. After a member has filled out the form it will be emailed to all the FTS instructors. The member should receive an email confirming the date for training. If members do not receive a confirmation email form the instructor please email FTS@fsxfleet.com and explain the situation.

 

CTS Training Request Form

Members who have recently been assigned to a cargo or other squadron may request training by filling out the CTS Training Request Form. After a member has filled out the form it will be emailed to all the CTS instructors. The member should receive an email confirming the date for training. If members do not receive a confirmation email form the instructor please email CTS@fsxfleet.com and explain the situation.

Members who have recently been assigned to a helicopter squadron may request training by filling out the HTS Training Request Form. After a member has filled out the form it will be emailed to all the HTS instructors. The member should receive an email confirming the date for training. If members do not receive a confirmation email form the instructor please email HTS@fsxfleet.com and explain the situation.

Training Report Cards

1FTS Training Report Card

Instructors who have just completed training with their assigned student are required to fill out a training report card. After it is filled out it will be emailed to the head instructors email address for approval. Once approved the student will receive a promotion to Paygrade O2 if not already promoted. The student will also receive a certificate of Training Completion and be awarded their wings.

Application Forms

Sign Up Form

The FSX Fleet Sign Up Form is for anyone in the FSX Community that is interested in joining a virtual military organization such as FSX Fleet. People interested must fill out the sign up form to get accepted into the FSX Fleet. Once a person has filled out an application they will be accepted by the FSX Fleet application manager. This process will take a few days and people will receive an email with the all the information needed when they are accepted into the FSX Fleet. Note: Please do not fill out multiple applications as it decreases your chances of being accepted.

 

Reinstatement Form

The FSX Fleet Reinstatement Form is for former members of the FSX Fleet that have been discharged, dismissed, or resigned from the FSX Fleet. Only a few of the members that fill out this form will be accepted back into the FSX Fleet. What is said on this form impacts the decision to let a person back in.

 

Operation Red Flag Application Form

The FSX Fleet Operation Red Flag Application Form is for FSX Fleet members who are interested in participating in upcoming operation red flag missions. After submitting this form applicants will be emailed by the red flag staff to confirm their position in the mission. This form will be updated 1 week prior to A Red Flag mission. Note: This is strictly for FSX Fleet members only.

Contact Forms

Webmaster Form

The FSX Fleet Webmaster Form is used to contact the website administrators and FSX Fleet Chief of Staff himself. This form can be used for many reasons, some of which are technical support, complaints, questions, and comments. People using this form will be required to give the information required to contact them back.

 

Incident Report Form

The FSX Fleet Incident Report Form is used to report certain incidents that happened on or off the teamspeak server that involved one or more FSX Fleet members. Remember to go into detail on what is said on this form as it will determine whether an investigation is launched into the matter or not. Members will receive an email about the situation if the report is approved.

Other Forms

Officer Candidate School Completion Form

The Officer Candidate School Completion Form is for OCS instructors to fill out. Once a person is taken through OCS the instructor is required to fill out this form. Note: If you are not an OCS instructor then please do not fill out this form.
